To promote good health and
well being in patients registered with the Phoenix Surgery, who are aged 75 years
and over, with the help of trained volunteers.
The Staywell 75+
scheme is a well established early warning system set up by a group of family
doctors to help keep older people well.
Health problems often increase as we get older, but an early warning of difficulties
may help to prevent the longer term problems; the Staywell 75+ scheme is coordinated
by a health visitor and aims to pick up on these early warning signs to help
prevent any longer-term difficulties, where possible.
The first signs of illness among older people are often associated with an inability
to manage the ordinary activities of daily life. The Staywell 75+ team uses postal
questionnaires and regular, confidential interviews with patients in their homes
to identify any problems which can then be followed up accordingly.
Currently there are also over 20 volunteers, trained and
supported by the Health Visitor for Older People and Staywell Administrator,
who add a valuable element of befriending to patients.
The scheme is supported by the Phoenix Charitable Trust, Cirencester and District.
